Objective

To explore the effectiveness of autologous peripheral blood-derived cells, combined with hyaluronic acid, for the repair and regeneration of chronic Achilles tendinopathy (CAT), a condition known for its poor healing and lack of curative treatments.

Results

  • Three patients with long-standing Achilles tendon pain (ranging from 1 to 5 years) underwent surgical intervention involving multiple needling of the tendon, followed by repeated intra-tendinous injections of peripheral blood-derived cells and hyaluronic acid.
  • All cases showed significant improvement in pain, tendon structure, and function, as measured by the VISA-A score (a validated assessment tool for Achilles tendinopathy).
  • MRI scans confirmed a reduction in inflammation and tendon thickness, showing signs of structural regeneration.
  • No adverse effects were reported. Two patients achieved near-complete recovery, scoring 97 and 100 on the VISA-A scale.

Conclusion

This small case series demonstrates that peripheral blood-derived regenerative cell injections, in combination with tailored physiotherapy and minimally invasive procedures, may offer a promising biologically based treatment for chronic Achilles tendinopathy. The method appears safe, effective, and sustainable over the long term. Further clinical trials are recommended to validate these early results.
